I've joined through a couple of different sites and sooner or later you get most sites. The main difference is the order in which you get them and the price you might have to pay. I joined through gapeland and I do have access to 52 sites instead of the more common 40 but most aren't updating anymore or they offer leased content.
If you want access to club sandy, zoliboy or pix and video than join using one of these sites because none of these are open to me. The good thing about looking at any other site than the main 21sextury is that at the bottom of each site is the breakdown of the site you will get and the order in which they become available.

For a period of time, they displayed the sites in order by when you got access - and even had notes on them on the home page "Instant Access", "Loyalty Bonus", etc to give you a pretty comprehensive idea of what you got access to when before you joined.
They don't do that any longer - but, I believe that you now get access to all of the Premium Sites - PixandVideo.com, ClubSandy.com and Zoliboy.com - when you join, which was not the case when they had the former arrangement - and I believe that you have access to all sites by the 3rd month of your membership in the current set-up, where they used to dole them out 2 sites at a time.

I wondered the same and posted the question in the forum under the title "Need some advice on 21Sextury please" I don't know how to link the thread so you'll need to search the furum for it. I got some answers, but I have no idea if that may have changed in the couple of months since I asked the question.
I find their pricing model to be confusing...
